The Mayflower Medical Practice can offer a range of travel vaccinations if you are going on holiday or travelling abroad

Please contact the practice several weeks prior to your intended foreign trip/holiday so that any necessary vaccinations and immunisations can be arranged with our Practice Nurse.

Once you have completed the form please return it to us.  The Nurse will then see what you require for your stay away and will invite you to attend for a Vaccination Appointment.

Please note that travel vaccinations need to be given 6 to 8 weeks before your travel. Please give us as much notice as you can prior to travel as some vaccinations require more than one dose prior to travel to ensure that you are protected to the recommended level. Travel vaccines that have to be ordered specifically for a patient may incur a cost and must be paid for by the patient prior to receiving the vaccination. Costs will be discussed at the first appointment with the Nurse.

We can also supply Malaria tablets which incur a charge; unfortunately we do not offer Yellow Fever Vaccinations.  Information regarding where you can have this vaccine is available on request.
