Our practice list is currently open and we accept registrations from people who reside in our practice area. Please refer to the practice boundary map below.   

Catchment Area

The practice has an inner and outer boundary. If you reside in the inner boundary area, and are registered with us, and you move to live in the outer boundary area, you may be able to stay registered at the practice; however you must always contact the practice with your new address.

Boundaries are set to ensure patients who live in the boundary area receive the services offered by the GP practice.  If you live outside the outer boundary you will be asked to register at another practice in the area you are now living.

To register at the practice you will need to complete our Registration Pack.  Registration Packs are available on request from the receptionists at both sites, or can be downloaded from our website.  It will be necessary for you to attend in person at the practice to submit your completed registration documents and to provide photo identification and proof of address. Patients are registered with the practice and not an individual GP.

Please allow approximately two days for your registration to be fully processed, after which you will be required to book an appointment with our healthcare assistant for a new patient medical. Patients with an urgent medical need will be fast tracked through the registration process to access immediate medical care.

Our practice boundary includes Doncaster and Nottinghamshire areas; we are therefore able to provide patients with a choice of attending Doncaster Royal Infirmary or Bassetlaw Hospital where appropriate.


Registration

If you reside in our practice area and would like to register at the Mayflower Medical Practice please complete the following 3 PARTS of the registration process. Please bring all completed forms and your ID to the reception at either our Bawtry or Finningley surgery.

PART 1

Complete our ‘New Patient Medical Questionnaire’

You will be required to arrange an appointment to see one of our Healthcare Assistants for a New Patient Check.

PART 2

Complete the PINK GMS1 registration form

Please complete the form in full and hand in to our reception at either our Bawtry or Finningley surgery.

PART 3

Provide suitable Photo ID and Utility Bill with Address to reception for verification.

We also accept Birth certificate, Marriage certificate, valid Passport, Medical card, Local authority rent card, Paid utility bills, Letter from Benefits Agency/benefit book/signing on card, Papers from the Home Office.

Alternatively please ask at reception for our Registration Pack.

All patients who are registered at the practice will be allocated a named accountable GP.

The information on the medical card (if you have it) will speed up the process of getting your records from your previous practice. Once you have been accepted as a patient your medical records will be transferred to us and a new medical card will be sent to your home address. You do not need to inform your existing practice that you are moving, although it may be courteous to do so.
