How to apply for registration:
To access our Online Services you need to first register for the service. To do this you must bring photo identification into the practice and speak to one of our dispensers or receptionists who will ask you to complete the appropriate application form. They will provide you with a secure password and login and instructions on how to use the service.
To access additional online services, ie detailed coded records, you must complete the appriopriate application form and provide proof of ID even if you have already registered for online services with us.
Please ensure you read the Guidance Notes before completing the appropriate form:
- Adult form for patients over 16 years of age
- Child form for patients under 11 years of age for completion by parent
- Proxy form for patients to give permission to the Practice to allow their chosen representative to have proxy online access to their records

Access to detailed coded GP records is available from 1st April 2016 and upon approval by the GP.

Online Records Service
To access our Online Services you need to first register for the service. To do this you must bring photo identification into the practice and speak to one of our dispensers or receptionists. They will provide you with a secure password and login and instructions on how to use the service.
Accessing Summary Care Records
From the 1st April 2015 patients will have the ability to access their summary care record. The summary care record will include a summary of information relating to medications, allergies and adverse reactions. As well as viewing this information there will be the facility to download and print it.
To access your summary care record you must register for this service at the practice as explained above.

Accessing Detailed Coded GP Records
From 1st April 2016 patients will be able to look at their detailed coded GP records. As well as viewing this information there will be the facility to download and print it. To apply for this particular service you must complete the appropriate application form and bring in to the practice reception team along with your ID. Patients who have already registered for Online Services will need to apply for access to their detailed coded GP records as explained above.

- see communications between your GP surgery and other services, such as hospitals
