Care Navigation is a person-centred approach that uses signposting and information to help primary care patients and their carers move through the health system and social care system as smoothly as possible to ensure that unmet needs are met. It enables frontline staff to provide patients with more information about local health and wellbeing services, both within and outside of primary care, in a safe, effective way.


The Receptionists are here to help you. Their responsibilities include arranging appointments, taking and relaying messages, arranging repeat prescriptions, accepting requests for home visits, following the Doctors' instructions to discuss appropriate information to patients. Please remember the receptionists are acting on the instructions of the Doctors. The more information you can give about the nature of your problem will help us to book you your appointments with the appropriate healthcare professional.
